Position Overview

The information below provides a snapshot of what this position will be responsible for.

Provide telephonic patient-centered case management and health coaching services for the early detection and management of chronic kidney disease, diabetes, and other underlying comorbidities.

Essential Job Functions

The individual in this position is expected to engage in the following work-related duties:
  • Gain thorough understanding of the CKD program and its benefits; explain the CKD program to new and/or potential members in a manner that encourages them to utilize the program to improve their health and quality of life.
  • Appropriately document all outreach efforts and use this information to improve your approach and strengthen our ability to retain members.
  • Manage a caseload of up to 300 members; ensure each member is treated with the respect every human deserves.
  • In a personable manner, collect health assessment information from members. 
  • Use individualized, goal directed and patient centered health plans to empower members to take control of their health and therefore improve their overall quality of life.
  • Accurately document member cases which includes inputting individual health data and identifying potential cost containment areas.
  • Work collaboratively with each members’ health care provider to ensure they understand the CKD program, know how to/can access various our support options, and are empowered to utilize the support and information provided to offer the best possible care. 
  • Work collaboratively with each member and their support system to ensure all questions and concerns are addressed quickly and in a manner that fosters continued engagement and utilization of the program. 
  • Adapt to the needs of the position and work autonomously to meet those needs while also proactively collaborating with and seeking help from colleagues and supervisor as needed.

Talents and Strengths Requirements

  • Must be an LPN/LVN.  
  • Must have at least 3 years in a related role.  Experience with working as a remote Case Manager is preferred.
  • Must possess a current and active nursing license in good standing with the issuing state.
  • Ability to learn and follow established processes; provide suggestions to improve these processes upon request or as needed.
  • Ability to input, track, and query stored data; utilize data to inform and support work related efforts. 
  • Ability to communicate with others in a personable, engaging, and professional manner in person, over the phone, and via email. 
  • Strong working knowledge of common office software such as MS Office, Visio, and CRM platforms; ability to learn new software quickly.

  • Ability to pick up, lift, assemble, carry, and move objects and equipment of up to 30 pounds.
